Use this worksheet to request and organize vendor facts before HR/legal/counsel review. Not legal advice.
Purpose: get the facts needed to decide whether a hiring tool belongs in the AI hiring compliance file. Do not treat vendor marketing language as proof. Save source dates, documents, and reviewer notes.
1. AI/automation descriptionAsk whether the product uses AI, automation, ranking, matching, recommendations, scoring, summaries, or decision support in the employer's hiring workflow.
2. Employment decision roleAsk which workflow is affected: recruitment, screening, hiring, promotion, discipline, discharge, training selection, tenure, or employment terms.
3. Data inputsAsk what applicant, employee, job, performance, assessment, interview, or behavioral data is processed.
4. Human oversightAsk whether humans can review, override, disable, configure, or audit the tool's recommendations or scores.
5. Bias/adverse-impact materialsAsk what validation, bias testing, adverse-impact analysis, audit summaries, or methodology notes exist.
6. Change controlAsk how model, feature, data-source, scoring, or workflow changes are documented and communicated.

Confirmed: supported by employer or vendor evidence. Public signal: visible from public materials only. Client-reported: reported internally, evidence pending. Unknown: needs vendor or responsible-party confirmation. Counsel review: legal sufficiency or business/legal decision required.
